“Who’s brave enough to retain this tooth?”

A middle aged male in overall good health presented with generalized mild to severe chronic periodontitis requiring full mouth therapy.  Teeth 24 &25 demonstrated severe bone loss.  The bone loss on #25 appeared to extend beyond the apex however it tested positive to Endo Ice. The initial assessment was that teeth 24 &25 would eventually need to be extracted but if they were treated prior to extraction, significant peripheral bone regeneration could potentially be obtained.  Teeth 22-26 were splinted for stability prior to treatment.  The LANAP protocol was performed and the initial evaluation was done at 4 months.  Surprising bone fill was documented radiographically.  Probing depths were 4mm or less.The patient was seen again at 7 months and additional ossification was noted.   Both teeth remained vital.  The initial splinting was intact and the teeth were functional.
